Nursing care at home

Phoenix Healthcare Nurses can provide short-term and long-term nursing services for general and complex needs.

Our services are delivered under Integrated Home and Community Support, and ACC contracts.

We have Registered Nurses and Enrolled Nurses with a wide range of experience including accident and emergency, mental health, and primary health, who can provide patient centered specialist nursing care in your home.

The range of services include:

• Comprehensive care planning
• Wound assessment, treatment, and management
• Medication management and oversight
• Catheter care and support
• Continence support
• Subcutaneous and intramuscular injections
• Spinal/Ventilator care

We offer supports that are flexible and responsive to your needs and are backed by an on-call nurse available 24 hours a day, 7 days a week.

Referrals

Access to Phoenix’s Community Nursing services is via referral from:

• Your GP
• Public & private hospitals.
• Community health support agencies (NASC or ACC Coordinators)
