Your key role | Tō tūranga matua

Phoenix Healthcare is on the lookout for full time Registered Nurse who embodies confidence, is passionate and has sound knowledge and understanding of diverse needs, including people with disabilities, older persons health and district nursing. This role is perfect for a Registered Nurse ready to make a substantial impact through direct community engagement.

This role is full time with 0.6 Community Nursing and 0.4 Clinical training to support workers to meet their competencies.

  • Conduct comprehensive care plan assessments in the community, ensuring each client’s health needs are meticulously identified and addressed.
  • Carry out initial evaluations and follow-up reviews, establish care goals, develop detailed support plans, deliver ongoing care and treatments, monitor client progress, and conduct clinical assessments during community visits.
  • Provide support, education and training to our home and community support workers.
  • Perform interRAI evaluations to determine the best care and support strategies for clients, fostering their health and well-being (training supplied). 
  • ACC wound care dressing, assessment, and evaluation
  • Utilise your clinical skills and knowledge to provide personalised care, adapting to the unique needs of each whānau.
  • Work autonomously, applying your expertise to make informed decisions and deliver high-quality nursing care.
  • Collaborate with a multidisciplinary team ensuring holistic approach.

The hours of work for this role are:

  • 8:30 am – 5:00 pm Monday -Friday

 

About you | Mohou ake

  • Registered Nurse who currently holds a valid practicing certificate in accordance with New Zealand nursing regulatory requirements.
  • Previous community or aged-care nursing experience is beneficial but not essential.
  • Display confidence in your clinical abilities, equipped with practical judgment to effectively use these skills in a community setting.
  • Be committed to Te Tiriti o Waitangi with knowledge and understanding of tikanga Māori
  • Proven interpersonal and communication capabilities, enabling meaningful connections with clients and their whānau.
  • The capacity to practice autonomously, efficiently managing your tasks and making informed decisions that have a beneficial impact on client care.
  • Flexible and adaptable, willing to travel across Canterbury region to meet client needs.
  • Possession of a full, clean driver’s license, coupled with the willingness to undertake travel within the community to meet the needs of our clients.
  • Legal right to work in New Zealand.
  • Ability to pass a Police Check.

This role is all about stepping in when the team needs you most, covering shifts for unexpected absences or when someone is sick. You’ll be there for our clients, who range from needing just a little support to those who rely on Phoenix Healthcare a lot. If you are a healthcare professional and you are looking for a role that fits nicely with your life, offering steady hours that help you keep work and life in balance, then this is right up your alley. This role involves being the go-to support across Canterbury region, stepping in to cover shifts wherever needed.

The hours of work for this role is:

  • 7:30 am – 2:30 pm (Wednesday, Thursday, Friday, and Saturday) Guaranteed 28 hours per week

On Saturdays, you will be on call from home, ready to respond to any instructions from the Coordination Team Lead. Meanwhile, on Wednesdays, Thursdays, and Fridays, you will report to the office and remain on standby throughout the shift period.

As a Support Worker on the Permanent Part-Time Reliever shift, your role offers a great deal of diversity since you will be assigned to work in various clients across Canterbury according to the roster. In environments where maintaining a routine is essential, you will offer personalised support, adhering to detailed care plans designed to cater to the individuals’ needs.

  • Must have a minimum of Level 3 in Health and Wellbeing qualification.
  • Have a full driver’s license and reliable transport.
  • Experience with complex care patients is desired but not mandatory.

  • Have excellent communication skills in English.
  • Possess strong interpersonal skills and integrity.
  • Be passionate about empowering clients.
  • A current First Aid Certificate is an advantage (we can provide certification).
